Applications&nbsp;cannot&nbsp;be processed without these documents.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Birth Certificate (photocopy)&nbsp;or&nbsp;<strong>Affidavit<\/strong>&nbsp;(please contact Registrar for more information: (413) 863-7507)<\/li>\n\n\n\n<li>Proof of Residency (See page 1 of the Student Registration Form.)<\/li>\n\n\n\n<li>Immunization Records<\/li>\n\n\n\n<li>Physical Examination (within one year prior to entrance to school or within 30 days after school entry)<\/li>\n\n\n\n<li>Lead Test (date and result) (for kindergarten and Pre-K registration)<\/li>\n\n\n\n<li>Student Records Release&nbsp;Form (See page 7 of the Student Registration Form.)<\/li>\n<\/ul>\n\n\n\n<p>If you have any questions about the registration process, please contact the Pupil Services Department at (413) 863-7507.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Immunizations<\/strong><\/h2>\n\n\n\n<p>We require all students enrolled in Gill-Montague schools to be immunized in accordance with Massachusetts state law.
