Dear Parent or Guardian,

As the school year is coming to a close, I have a few friendly reminders from the Nurse’s Office.

First, I would like to introduce myself. I was hired this past March to fill the position of the Sheffield School Nurse. I have been busy working over the past few months getting acclimated to the school and getting to know your amazing students.

Here are a few things to get started:

  • All students entering the 4th grade will need an updated physical this coming school year. Please call your student’s pediatrician to schedule a physical as soon as you can.
  • All students who will be taking medication during the upcoming school year will need a Medication Order Form completed by the student’s doctor. In addition, a parent/guardian will need to fill out a Permission to Administer Medication Form in order for the nurse to administer the medication(s).
  • All asthma and epinephrine medications to be administered in the coming school year will need an Allergy and Anaphylaxis Action Plan per a doctor’s orders and signed by your student’s pediatrician.

All forms can be picked up in the Health Office.
All medications, including inhalers and epi pens will need to be picked up prior to the end of school year.
